A deposit of $175 is due within five days of making your reservation. Final payment is due April 27, 2010.

Government taxes and fees are $118.85 per person. Single/Third Person Occupancy available.

If you are a pulmonary patient, also complete the 'Pulmonary Cruise Client Information' form and have your physician complete the 'Medical Information from Your Physician' form.
